Our Housing Process
Our process focuses on our core belief and philosophy that the personal, physical and spiritual well-being of our clients must come first. We take an inter-disciplinary approach to our management as well as a team approach to our service offerings. We partner with numerous local and state non-profits as well as governmental entities in order to best meet the needs of our community.
Application and Screening Process
Unlike most providers that have a check-list of requirements, we prefer to take a holistic approach. Most of our clients typically originate via a shared referral source. Often times this could be a doctor, nurse, case worker, social worker or even another clients’ family. Doing so allows our qualified staff to better understand the special needs of the particular client and therefore make a decision.
All of our Transitional homes are specifically designed to provide housing and related services for each particular demographic. Because of that, our residents have access to a wide variety of services provided by outsourced 3rd parties. These include but not limited to:
- Doctor / Nurse home visits
- Home health services (RN, Physical, Occupational and Speech Therapy)
- Durable medical equipment
- Case management
- Social work
- Behavioral counseling and therapy (if requested)
- Medication management
- Many additional services
Room & Board
The needs of our attendees will always come first. We understand that the people we serve are our customers and like all other businesses we want to provide a really good service. We want to go above and beyond to help provide a good foundation that will allow our attendees to focus on all aspects of their life that will help them improve and continue to move forward.
Note: Each home may cater to a particular demographic. We may also have homes available that are gender or age specific such as shared living home for Seniors.
